The plan covers employees, family members, and dependents.
The policy is available only in Maryland.
Employees can choose between a 10% annual payout or an annual maximum of up to an additional $3.50.
Their spouse and dependent child can opt in for an additional 10% payout.
Employees will receive the full payout, but it will be capped at $4 million, which means they will receive an annual $6,500 maximum payout.

Employees in Maryland can also get a 10 year annuity for up the maximum payout amount, which is $24,500.
